ISSUES SUBMITTING A QUESTIONNAIRE

 

Please refer to the following if you are having trouble submitting a questionnaire:

POSSIBLE ISSUE:

Pop-up blocker is interfering with submitting questionnaire

SOLUTION:

Make sure all pop-up blockers are turned off

 

POSSIBLE ISSUE:

There is information missing from a required field

SOLUTION:

Verify all required fields/questions have been answered

 

POSSIBLE ISSUE:

The session has timed out after 20 minutes of inactivity

SOLUTION:

Close all windows you are working in and re-open the questionnaire by clicking on the link in the email you received

 

POSSIBLE ISSUE:

General system issue or technical problem

SOLUTION:

Close your browser and other applications that maybe running then re-open and re-submit your questionnaire

 

In the event you are still experiencing trouble, please try restarting your entire system.
